Awareness and understanding of medical errors have expanded rapidly during the past decade. The patient safety movement works to promote safer healthcare through system solutions; however, diagnostic errors have received relatively little attention. Diagnostic errors are estimated to occur in 10% to 15% of patient encounters. Cognitive errors contribute to over half of diagnostic errors and are associated with significant morbidity.
