COURSE DESCRIPTION:
Platelet-rich plasma (PRP) therapy is increasingly used in the management of osteoarthritis, tendinopathy, and other musculoskeletal conditions; however, variability in PRP preparation and reporting can make it difficult to determine the actual platelet dose delivered to patients. This educational activity will provide a practical framework for understanding and calculating the absolute platelet dose delivered by a PRP preparation based on whole blood volume, baseline platelet count, and platelet recovery rate, while emphasizing why device name, processing protocol, or reported platelet concentration alone may not adequately define the delivered dose.
The presentation will review evidence-based platelet dosing thresholds for intra-articular and tendon or soft-tissue applications and demonstrate how delivered platelet dose can be incorporated into clinical decision-making. Participants will also examine conflicting PRP clinical outcome data by considering differences in platelet dose and will discuss the limitations of current dose-response evidence, including the lack of a dedicated dose-response randomized controlled trial in rotator cuff tendinopathy and the potential influence of operator technique and patient selection on reported outcomes.

Orthopedists, internists, family medicine practitioners, emergency medicine physicians, urgent care physicians, podiatrists, physician assistants, athletic trainers, nurse practitioners, residents, medical students and all other interested healthcare providers.
LEARNING OBJECTIVES:
- Calculate the absolute platelet dose delivered by a PRP preparation from whole blood volume, baseline platelet count, and platelet recovery rate — and explain why device name, spin protocol, or fold-increase reported alone does not define a dose.
- Apply evidence-based dosing thresholds to clinical decision-making — ≥10 billion total platelets for intra-articular knee and hip osteoarthritis, and ≥3.5 billion per injection for tendon and soft-tissue targets including rotator cuff, gluteal, and lateral epicondyle.
- Appraise conflicting PRP outcome trials by stratifying them according to delivered platelet dose, and identify the limitations of the current dose–response evidence, including the absence of a dedicated dose–response RCT in rotator cuff tendinopathy and the confounding of dose by operator technique and patient selection.
