Baptist Health Medical Group: Evaluation and Management Provider Onboarding Documentation Course
This course provides Baptist Health Medical Group practitioners with requirements for accurate Evaluation and Management (E&M) documentation and coding to meet requirements and adhere to CMS guidance. Completion of all three modules is required, with a passing score of 75% or higher. This course is an onboarding requirement for Baptist Health Medical Group healthcare providers.
Target Audience
Baptist Health Medical Group Physicians, Nurse Practitioners, Physician Assistants, and Athletic Trainers.
Learning Objectives
- Accurately identify the required documentation for the CMS determined key components and contributing factors that support the E&M level for patient visits in both the outpatient and inpatient setting.
- Implement strategies discussed to achieve organizational accuracy rate standard for E&M coding.

Patient History
- Implement elements of documentation of patient history as it pertains to CMS requirements.
Module 2
Documenting Your Examination
- Distinguish the differences between 1995 and 1997 examination CMS guidelines, and the documentation requirements.
Module 3
Medical Decision Making (MDM) & Contributing Factors to Documentation
- Adhere to the requirements and documentation guidelines for Medical Decision Making element of the CMS Guidelines.
- Recognize the impact 2021 CMS Guidelines changes will have to documentation practices.
