COURSE DESCRIPTION:

A multidisciplinary team approach to care is fostered through peer-to-peer discussion and collaboration. The team discussion pertaining to treatment includes recommendations for benign hematologic patients having complex medical management to bridge gaps across the continuum of care and enhance the overall quality care. Examples of medical management includes, but not limited to anticoagulation, thrombocytopenia, renal failure, lupus, and other conditions. During these case studies, participants will optimize standard of care and clinical harmonization between Miami Cancer Institute and Lynn Cancer Institute. 

TARGET AUDIENCE:
Hematology Oncologists/Medical Oncologists, Surgical Oncologists, Radiation Oncologists, Advanced Practice Providers (APPs), Pharmacists, Nurses, Social Workers, and other healthcare personnel involved in the care of benign hematology patient.
 
EDUCATIONAL OBJECTIVES:

•    Discuss clinical decisions and optimize appropriate referrals and testing for patients to transition between inpatient to outpatient    continuum of care. 
•    Develop comprehensive care plans of complex clinical management of patients.
